The following form was created to assist insurance companies to comply with the notification of incendiary fires, as required by North Carolina General Statute 58-79.40(c).  This form can be used in lieu of a written letter.
 
Upon receipt, a copy of the notification will be sent to agents assigned to the area of the incident.

The following form was created in order for citizens to provide information to assist with the investigation of intentionally set fires.  Information can be submitted anonymously, but you can include your information, if you wish to be contacted by an investigator.
