This will be all authorized personnel at your facility. The last line on this form is asking for your NCSBI’s Access Agreement number/OCA/ORI please leave this blank. Your agency number will be assigned to you upon final approval.

NCSBI ask that agencies maintain the Regulations Forms for Authorized Personnel at their agency after completion. This form should be kept on file to be provided during the agency’s Noncriminal Justice Audit. NCSBI will audit Noncriminal Justice agencies every three years; at which time an auditor will review the agency’s CJIS Online account to determine Authorized Personnel and verify the completion of CJIS Security Awareness Training. Individuals listed within CJIS Online will be considered Authorized Personnel and a copy of each individual’s Regulations Form will be required during audit. As changes occur, the agency’s CJIS Online Administrator should update the individuals listed in the account to reflect the agency’s current Authorized Personnel.

The NCSBI will establish the NCJ agency’s account in CJIS Online, a CJIS Online Administrator must be identified.  The Administrator is designated during the agency onboarding process.
