*Effective February 1, 2020, the State Bureau of Investigation will no longer be providing blank FD-258 applicant fingerprint cards. For fingerprinting needs associated with a background check, please contact your local law enforcement agency. Please see memo for further information.

Agency Privacy Requirements for Noncriminal Justice Applicants

Privacy Act Statement - EACH APPLICANT MUST BE PROVIDED A WRITTEN COPY OF THE FBI PRIVACY ACT STATEMENT
  • Applicant/Employee must take both forms (the Electronic Fingerprint Submission Release of Information Form AND the Applicant Information form) to the Law Enforcement Agency (LEA) to get fingerprinted. Prints will be submitted to the NCSBI electronically, only if the LEA has livescan capabilities.

  • If the LEA does not have a livescan device, a card will be printed and handed to the applicant to submit to the NCSBI by mail.
    Please verify all required information is printed on the fingerprint card. Completing a fingerprint card form

  • Once prints have been obtained, the applicant/employee must forward the two forms back to their employer.
    Do not mail forms to the SBI.These forms must be kept on file for one year at the corresponding agency for audit purposes.

Applicant Information Form

  • Employee completes all personal information

  • Employer completes: BOLD BOX [agency name, address, reason fingerprinted and agency number]
    Your agency number is referred to as an OCA number on the FD-258 applicant card. This identifying number may be referred to as an NCSBI Agreement number, an ORI number, or an Access Agreement (AA) number. This number allows us to link the background check results back to the correct agency.
